Aged Care Services Navigation

Assisting clients navigate the My Aged care system and register for home care services to assist clients remain independent in their home wherever possible.
Assist clients communicate with assessment teams and service providers to promote efficient and timely supports being introduced.
we have established relationships with most major Home Care Package “SaH” providers in Tasmania and self managed providers, EOL services are often able to be funded by SaH packages following discussions with your provider

We are supportive of an individuals right to request Voluntary Assisted Dying “VAD”, provide emotional support to those considering and applying for the service. Are able to provide information around VAD to community members as prescribed in the Act

Pet Support

As a fur baby parent, I understand the emotional impact of losing a beloved member of the family. I offer emotional support to those preparing to say goodbye, support with managing vet appointments and burials by request.

Educating community at ‘Grass Roots” level, applying a ‘Compassionate Community” approach
Community education around end of life choices
We host free community education events during Dying To Know Day “D2KD “, during Palliative Care week (pic of PCT talk panel) and Advanced Care Planning week. We speak at group gatherings like School for Seniors and PROBUS, often invited to community based expo’s where we will set up a stall to shed light on the work of doula and pretty much wherever else we are invited along to.
